Workplace Injury
In the event of an on-the-job accident, injury, or illness at the University of Pittsburgh, the injured employee and his or her supervisor should follow the guidelines provided here. In general, University employees should report all on-the-job accidents, injuries, or illnesses to their supervisors.

Reporting On-The-Job Injuries, Illnesses, or Accidents
Bloodborne Pathogens and Contaminated Sharps
If the on-the-job injury involves a bloodborne-pathogen exposure (such as a needlestick, contaminated sharp, blood/body fluid splash, etc.), medical treatment must be obtained by:

Also, the OSHA Bloodborne Pathogen Standard requires additional reporting for these types of injuries. A Sharps Injury Report Form must be completed by the injured employee's supervisor or departmental designee in addition to Forms 1, 2, and 3 required for reporting all on-the-job injuries.
Complete this form online or print it and fax the completed form to the Department of Environmental Health and Safety (EH&S) at 412-624-8524. EH&S compiles these reports into a Sharps Injury Log for the recording of percutaneous injuries from contaminated sharps as required by OSHA.
Animals or Animal Facilities
If the on-the-job injury involves an animal or occurs in an animal facility, medical treatment must be obtained by:
